High Point Home Health treats every referral with the upmost importance. We work closely with the Discharge Planners , Physicians and Case Managers to provide a smooth transition into Home Health Care services from almost any transition point. Our staff is very knowledgeable of federal and state guidelines consistent with acceptance and admission of all referrals. Our goal is to aide in a speedy Discharge to home or initiation of new home services even without a hospital Discharge.

We work with Discharge Planners and take responsibility to assure all needs are met upon return home of the patient such as any needed supplies or equipment.

The Registered Nurse, Therapists, Personal Care Assistants and Office Staff work as a team with Patients, Physicians and Case Managers to provide ongoing feedback of any patients need, changes and overall status.
